By signing up for any fitness, breathwork, or related activity (referred to as the "Activities") hosted by FRQNCY LLC and/or Meghan Gifford (the "Provider"), I understand and agree to the following:

Services and Disclosure:

I understand the Activities offered by the Provider are designed to enhance quality of life and support well-being and are not intended to constitute medical advice or any substitution for medical care. I understand that the Activities are not intended to be relied upon for diagnosis or treatment in relation to any health problem, and services of Provider do not replace the care of licensed professionals.

Assumption of Risk and Contraindications:

I acknowledge that participation in the Activities involves inherent risks, including physical injury, strain, discomfort, and potentially serious injury or death. I understand that the Activities may include strong connected breathing, which can lead to intense emotional and physical responses. I recognize that certain medical conditions make participation unsafe, including but not limited to cardiovascular disease, prior heart attack, high blood pressure, use of prescription blood thinners, epilepsy, glaucoma, osteoporosis, severe asthma, bipolar disorder, schizophrenia, dissociative disorders, history of significant trauma, and pregnancy. If I have any of these conditions, I will not participate. I acknowledge that the Provider is not qualified to assess my fitness for participation and that I must seek medical advice if needed.

Waiver and Release:

I waive any claim against the Provider for injury or damages resulting from my participation in the Activities. I release the Provider, its affiliates, employees, successors, and assigns from any and all claims, damages, liabilities, and expenses arising from or related to the Activities, including those resulting from the Provider's negligence. I assume all risks associated with the Activities, known or unknown, and take full responsibility for my participation.

Photography and Video Release:

I give permission for the Provider to use photographs and videos of me for promotional purposes without compensation.

Acknowledgment:

I have read and understand this Waiver and agree to its terms.

By signing up for the Activities, I agree to release the Provider from liability and assume all risks associated with participation.
